He was born on September 7, 1964 in Council Bluffs to George and Mary Jo (Bellamy) Collins. Thomas graduated from Thomas Jefferson High School.
He was preceded in death by his father, George.
Thomas is survived by his mother, Mary Jo; sister, Lisa (Will) Dang; brothers, David (Lori and James (Daphane) Collins; many nieces, nephews and family.
Graveside Service: 3:00 PM Saturday, March 18, 2023 at Crescent Cemetery.
